Although she's certainly the most important woman in Denji's life along with Makima, Power ranks last on the list of romantic interests for one simple reason: they're brother and sister in every way. After the International Assassins trial, the two were allowed to have intimacy, but Denji was way over it, and because he was able to connect with Power emotionally without it being romantic.
They shared baths and even slept together, not because of lust, but because they were two damaged children who were afraid of the dark. Denji himself openly understands that he loves Power, but without romantic feelings at all, and that their spark is not physical. It's the only relationship that made Denji selfless; a platonic love that defined his new vision of what it means to love someone without reciprocation.

Makima
Makima is the epitome of toxic manipulation. For her, every person is just a means to an end. She was never in love with Denji, but with the Chainsaw in him. Denji was just a guide for her to stick to her idol. She took advantage of his starving instinct, providing him with a normal life and kindness, only to methodically destroy everything he had ever loved. Her romance was a masterpiece of nurturing and mental torture, designed to destroy his spirit so she could possess the Hero of Hell. It's the weakest bond Denji has ever had because it was based on a lie – a parasitic slavery where he was under the impression that he was a dog and not a human.
Himena
Himeno is a complex character in Denji's life. As a seasoned hunter who had lost too many partners, she needed a relationship more than the need to attend another funeral. Her “interest” in Denji was quite unethical considering the age difference and her true love interest, Aki, was too blinded by revenge to even notice.
But despite the red flags, she was one of the very few people who treated Denji like a human being. She didn't want his heart as a mission, but as a companion to help her forget the “Easy Revenge” that had consumed her life. If they were the same age, she might have been the only woman to ever show him steady, civil love, even though it may have been followed by excess cigarettes and mutual trauma, but hey, that's part of the magic of being in love.

Kobeni Higashiyama
The fact that Kobeni is included as a love interest is almost coincidental, at the behest of Pochita's version of Chainsaw Man. The date they went out on was a nightmare scenario made surreal as she was forced to participate in a mock-up of an ordinary outing in a state of fear for her life.

To be honest, Kobeni is a literal reflection of Denji, but in an abstract way. No one really understands the plight of the victim of the evil world because she is the only one who knows what it's like to be a chess piece. Although the romantic chemistry was never there, their time together at the burger shop was a rare case of two individuals who were in each other's misery and didn't have to go through the grand schemes that Denji is used to when dealing with women.
Fumiko Mifune
Fumiko is another type of horror in Denji's life: an obsessive admirer. She is a devoted follower of the Special Division of Public Safety and claims to be a big fan of Chainsaw Man to the point of defending Denji in her own twisted way. Her interest isn't stronger than Makima's, other than the fact that she seems to like the chaos Denji causes, but it's superficial. She is attracted to the “hero” and “legend” and often overlooks the real teenage boy trying to support a pack of dogs and a little sister. It serves as a reminder that while Denji is popular, it's usually not for the right reasons.

Asa Mitaka
It was with Asa Mitaka that Denji finally encountered something that could be considered more of a true high school romance, whatever the mess. Both are outcasts in society and don't know how to function in normal society. Their aquarium date was a nightmare of social anxiety and awkwardness, but it was real and their connection was genuine.
Asa didn't come to Denji like Makima or Reze, first as a trained agent or manipulator, but only as a lonely girl looking for someone to bond with. Their relationship is based on the energy of each other's loners, making it one of the most reliable and powerful romantic possibilities in the show, as it's based on two people struggling with how to be human together.
Yoru
The way Yoru cares about Denji is inseparable from his care about Asa, but it has its own weight. As a war fiend, he is tasked with killing him, but when Asa is brain-linked, he experiences the same heartbeat and true attraction that Asa does. This makes it an interesting and high-stakes game in which interest is literally a struggle between the desire to destroy and the desire to love. Yoru's emotions are strong because they are beyond her control; she is a horseman of the Apocalypse and is corrupted by the most human emotions of the girl she inhabits, and their relationship is the strongest and most dangerous in the manga.

Reze has been the most compelling love in Denji's life since she first gave him the option. Although her first intention was a mission to the Soviet Union, their time together, nightly baths, lessons in the cafe, and laughter together, this was the first time Denji had experienced a spark that wasn't about his chainsaw heart. Reze is a weapon who was a child soldier and never lived a typical life, and she saw someone like her in Denj.

The final evidence of her moods was her decision to return to the cafe at the end of the arc from the train station, knowing full well what she was about to face. She was the one who really fell the hardest, and it turns out that it's still possible to find a true heartbeat for another, even in a survival horror setting.
