Smile 2 is a direct and indirect sequel to the original film. Aside from the opening, which ties back to Joel from the first film, the main plot revolves around the return of the pop star. Skye had some substance abuse issues which led to the traumatic event.

Pumpkin Spice And Knife Splice
The movie opens with Joel from the first movie sitting in his car waiting to ambush a man with Pumpkin Spice Frappuccinos. He leads the man into the house to see what Joel believes to be the main gangster. A quick knife kill that's pretty visceral is followed by a shootout with a lot more mobs.
It's not exactly how Joel wanted it to go, as he probably planned to undo the curse. Instead, he has to hide from other mobsters who arrive after the men he attacked die. This intro shows that this is a much bloodier film than the original or other media and overall it doesn't work.

Got Pancaked By A Truck
While Paul didn't die in that mob house the way he wanted, he did indeed die shortly after. As he escapes, he runs into the middle of the road, where a truck absolutely destroys him. Other horror films may be content to show a trail of blood and leave death to the imagination, but this is not the case.

The audience soon learns that Skye, the main character, has been in a car accident. This killed her actor boyfriend Paul and left her with intense scarring on her back and chest. She also has crippling pain that won't go away and normal painkillers won't solve her problems.
Because of his sketchy past, he can't get anything harder than through drug dealers. When she runs out of good stuff, Skye calls her old dealer, Lewis. Lewis is hysterical when he goes there, which eventually leads to an episode where he sees some kind of horrible monster. Then he kills himself with a separate scale and they manifest all.

There is one super quick kill in the movie that they don't spend much time on. During this experience, Skye keeps getting messages from an unknown number. Because this murderous parasitic thing causes people to see illusions, the audience is led to believe that all of these texts are in Skye's head.
However, they are real, which leads Skye to a man named Morris. She is a nurse who wants to help Skye end this curse because it took his brother. As he tells her, the film cuts to Morris' brother removing his own jaw with a crowbar. What would make him use a crowbar if he didn't play a lot Half-life?

Destructive use of mirror shards
Skye's mother, Elizabeth, only seems to care about one thing Smile 2: Skye goes on tour. Even though he is behaving erratically, he chooses not to see it. This all comes to a head after Skye wakes up from the accident.

Whether it's an illusion or not, Skye goes through the wringer on stage when she faces some Elden Ringa level demon entered her terrifyingly stretched mouth and then stabbed herself with a microphone on stage. Her fans don't seem happy with their chosen concert.
