Starting on the fourth day Pathological 3you will take on a vital role in diagnosing patients suffering from intercurrent diseases – diseases that slow down the deadly sand bug. Accurate diagnoses are essential: each correct identification helps unlock the next day's vaccine, dramatically reducing infection rates and allowing you to advance further in the game.
We will break down the diagnostic process step by step, explaining how to collect symptoms, interpret biological samples and conduct field investigations. We've also included a full table of patient diagnoses for those who need help or prefer to focus on other aspects of the game.

How to diagnose patients
To successfully identify diseases:
- Start with a thorough collection of symptoms. Carefully examine each patient by checking dialogue options, inspecting their portrait for visible signs such as facial discoloration or lesions, and performing a body scan. Once you've identified potential symptoms and selected them in your casebook, discuss each one with the patient – any symptoms that open up further options for dialogue are confirmed indicators worth noting.
- Narrow down the options. After collecting all available symptoms, compare them with known diseases. If only one disease fits your list of symptoms, you've probably found your answer, but you'll often be forced to choose between several.
- For ambiguous cases, use biological samples. When two diseases share similar symptom profiles, biologic sampling becomes necessary. The key is to identify which organ systems are different in suspected diseases. Each disease's information panel will show you which organs are infected when the patient has them and which organs are healthy.
- Follow out-of-hospital investigation guidelines. Some diagnoses require the collection of evidence outside of clinical examination. Your patient's dialogue will tell you when it's necessary, and a marker will appear on your map indicating where to examine. These field surveys can reveal additional symptoms or help you eliminate false leads.
