Sarada Uchiha first awakened the Mangekyo Sharingan shortly before the Boruto series timeskip. However, at the time she was not really aware of the fact that she had gained this power. It was only after the timeskip in the Land of Wind that Sarada was able to truly awaken her powers and gain control over them. In the fight against Shinju Ryu, Sarada gained access to her Mangekyo Sharingan after witnessing Yodo's sacrifice. This allowed her to take her visual prowess to a whole new level and unlock herself her strongest jutsu, Ohirume.

Thanks to Ohirume's power, Sarada Uchiha can manipulate black holes and gravity. Sarada can use Ohirume to create up to 4 different black holes, the size of which she can control at will based on how much chakra she puts into it. At the same time, each black hole possesses an enormous amount of gravitational force. Sarada can freely control what will and will not be affected by this gravity. With this gravity of his, he can even make himself float. The gravity that Sarada emits is so strong that all of her enemies are completely powerless against her and are pulled into the core of the black hole. Sarada can then completely crush them inside the black hole and cause them to explode, reducing them to pieces.

In the battle against Ryu, Sarada used a unique attack to completely destroy him in battle. Last time, Sarada took on 16 different Mamushi clones in Konoha and completely destroyed them with a single move. It is obvious that Sarada Uchiha's use of Ohirume jutsu is quite incredible and it goes without saying that it is one of the most powerful jutsu that has ever existed in the entire history of Naruto. However, such a jutsu comes at an extreme cost, and that is her sight.

All Mangekyo Sharingan eventually cause blindness to the user, and that's no surprise. It goes without saying that he eventually needed an eye transplant from Itachi to get rid of this permanent blindness.

For Sarada Uchiha, the problem is something similar. After using Ohirume a few times, she is already starting to lose her sight. After Sarada Uchiha took care of 16 Mamushi clones in the recent Boruto chapter, Sarada Uchiha's vision started to blur, meaning she is already feeling the effects of using this power. He will eventually go completely blind unless he cures this blindness one way or another.

How will Sarada Uchiha avoid permanent blindness?
There are only two ways for Sarada to cure her blindness at this point. The first is for her to have another pair of Sharingan transplanted from a blood relative, giving her access to the Eternal Mangekyo Sharingan. Just like Sasuke looked at Itachi, Sarada would need someone's Sharingan, probably a blood relative, to get rid of this permanent blindness. The second is for her to become a healing ninja and then unlock the power of the Byakuga, just like her mother. This would then give her great healing properties to offset any damage from the Mangekyo Sharingan's power. As such, Sarada using her Mangekyo would not sacrifice her vision. For now, Sarada is still going blind without being able to completely get rid of it. In the future, however, he will surely figure out a way to preserve his vision.
