Azmodan's lore and role in Diablo 4 Season 11 explained

Azmodan in Diablo 3

Azmodan's early history and the fight against primal evils

When Tathamet, the second being that ever existed, was killed by the first being that ever existed, Anu, each of his seven heads festered and turned into godless deities known as the Lesser Evils. They are “lesser” only in the sense that they are individually weaker than the three Prime Evils; by mortal standards they are still immensely powerful.

Following the Prime Evils' attempts to gain humanity's allegiance during the Sin Wars, the Lesser Evils were dismayed and disillusioned, fearing that the Primes were distracted and cowardly in their war against Heaven. Azmodan worked with Diablo's Belial, another of the Lesser Evils, to overthrow the Prime Evils and banish them to the Sanctuary, the mortal plane of the in-game universe. The pair's main goals were twofold: to gain more control over Hell for themselves and to start a proxy conflict between the Primes and the forces of Heaven in the Sanctuary, thereby leaving the gates of Heaven open for attack.

Azmodan and Belial were successful, but soon turned against each other, throwing Hell into another civil war. Azmodan would have a considerable advantage over his opponent for a long time, but eventually they landed on some semblance of peace and once again set their sights on the mortals. Azmodan, hungry for more power, and not just Hell, launched an invasion of Sanctuary.

Azmodan's role in Diablo 3

Banished to Sanctuary and unable to manifest corporeally without a human host, Diablo, in the spirit of the Dark Wanderer, fathered Leah with the human Adria as an ideal vessel for the Primeval Evil. Raised by Deckard Cain, Leah grew up to aid the Nephalem in his battle against Azmodan, who was attempting to steal the Black Soulstone, a device used to trap demons. This journey would culminate Diablo 3's Mount Arreat, where Nephalem, Adira, Leah, and the fallen angel Tyrael confront and defeat Azmodan, trapping his spirit in the Black Soulstone along with the other six Lesser Evils.

But Adria, who was Diablo's double agent all along Diablo 3's story, he betrayed the group and sacrificed Leah to summon Diablo, who was now infused with the essences of Azmodan and his kin. Despite becoming the most powerful being in existence, Diablo was defeated by the Nephalem and his spirit returned to the Black Soul Stone, still filled with the souls of the Lesser Evils. Eventually, the Black Soulstone was destroyed and the Lesser Evils separated from Diablo.
