Withermoore's Soul Jar is a side quest Divinity: Original Sin 2 where players must find and destroy a phylactery created by Bracc Rex to trap the knight's soul inside. Play hide and seek with Mody in the caves.

Follow Mody through the ancient hatch to find Withermoore.

Use the shovel to dig up the dirt next to where you find the Mods the second time.

Talk to Withermoor, the impaled skeleton on the wall, to learn about his story.

Pull out the Spear of Braaccus Rex impaling Withermoor.

Ask Withermoor about his story and agree to help him find his Soul Jar in exchange for a path out of the fortress.

Fast travel to Fort Joy – Square Waypoint and interact with the Divine shrine to activate the button described by Withermoore.

Enter the Underground Vault that opens nearby and enter the Fort Joy Holding Cells.

Destroy or lock the door near the Waypoint and collect the Prison Key from the stool next to the Magister Houndmaster.

Use the Prison Key to enter Fort Joy Prison through the main door and follow the red flag marker on the map to find a lever next to a stone altar near the stairs.

Activate the lever and enter the hatch to enter the Ancient Passage.

Use telekinesis to place the vases on top of the vents, which spew Poison Clouds and block the passage.

Pick up Withermoore's Soul Jar above the ancient plaque that says “Here lies Withermoore the Supplicant”.

Return to Withermoore and either destroy Withermoore's Soul Jar (1600 XP) or say you won't destroy the Jar (1400 XP) to complete the quest.
Hide and Seek with Mods
Speak the child's name Mods in cavessouth of Fort Joy. He asks you to play hide and seek with him. Agree and follow him to his hideout to find him. After catching him once, he says he will hide again and this time he will be invisible. Go back to where you talked to him the first time (near Frolie) and he'll come out of stealth and say you caught him.
I'm talking to Withermoor
After you find Mody a second time, he'll say he wants to introduce you to a friend and go through a small crawl space nearby. Go up to the spot near the crawl space where the sunlight hits the ground and a dirt mound will appear. Use the shovel to dig up the pile to find the Ancient Stone Hatch. Go through the Ancient Stone Hatch to find Mody standing next to the large impaled skeleton of an undead knight named Withermoore.
Take out the Spear of Braaccus Rex, which is used to impale Withermoor to the stone wall (requires 12 Strength of 1 Telekinesis) and he will tell you his story. He reveals that he was imprisoned here by Braccus Rex after he was sent here to bring King Sourcerer to justice. Bracus Rex trapped Withermoor's soul in a jar and cursed him to stay alive for all eternity. The only way Withermoore can die is to destroy the Soul Jar, which he asks the player to do. In exchange, he says he can show you a way to escape Fort Joy.
Holding Cells
Agree to help Withermoore and he'll give you a clue: there's a secret switch on the Seven statue in the courtyard that leads to the main floor of Fort Joy Prison. Nearby is Braccus Rex's phylactery room, where Withermoore's Soul Jar is imprisoned.
Fast travel to Fort Joy – Square Waypoint and interact with the Shrine to the Divine. Select an option “Notice the button of the Withermoore described and press its worn surface. A small stone hatch to the underground will appear right next to the statue.
Enter the hatch to find Waypoint Fort Joy – Dungeon with a locked door nearby. Either attack the door to destroy it, or use the lock to unlock it and enter the Holding Cells.
Fort Joy Prison
At the other end of the Holding Cells is the room with the Magister Houndmaster. Next to him is on a wooden stool next to an overturned chair The key to the prison. You can either sneak up and grab the key, or kill the Magister Houndmaster and his lackeys to claim it.
Use the jail key to open the main door up the stairs and head inside Fort Joy Prison main area. Open the map here and look for the red flag on the map to find the stone altar of sa lever near. Pull the lever and the altar will give way, revealing another hatch that leads down to the Ancient Corridor.
If you kill him, be sure to grab the Shiny Red Ball from the Magister Houndmaster. Comes in handy during the Finding Emmie quest.
Ancient passage
Here you will see two big ones poisonous clouds blocking the way forward. Use telekinesis to do this move the vases on the sides of the room above the vents that spew clouds of poison. This stops the spread of the poisonous gas. Alternatively, you can use a fire spell to launch poisonous clouds and explode them, then run through the area without getting hurt.
If you have an undead party member like Fane in your party, they can move through poison clouds with impunity since it doesn't deal any damage to the undead.
Withermoore's Soul Jar
On the other side of the poison clouds, you'll find a large chamber with five soul jars labeled Withermoore's Soul Jar. Only one of these containers is real; the rest are traps. The ancient plaque of the real Withermoore's Soul Jar states, “Here lies Withermoore the Suppliant. This is the Jar found at the far end of the room on the left.
Two of the fake Withermoore's Soul Jars will summon undead enemies to fight you. For maximum profit, it's worth summoning these enemies to gain extra experience.
In the alcove, there is a statue accessible by a ladder in the Ancient Passage (near the Soul Jars) that contains Tyrant's step a piece of leg armor. This is part of the Tyrant set and is required to complete the Artefacts of the Tyrant side quest.
Destroy or keep Withermoore's Soul Jar
Collect the real Soul Jar and bring it back to Withermoore. Here you have two options:

Talk to Withermoore and say you will destroy the Soul Jar (1600 XP).

Talk to Withermoore and say you want save the Soul Jar (1400 XP). Withermoore becomes hostile (200 XP for killing him).
No matter which method you choose, Withermoore will die. If you are the one who ends up killing him, the Soul Jar in your inventory will become “Empty”, indirectly granting Withermoor's wish.
To get the most experience, first tell Withermoor that you won't destroy the Soul Jar (1400 XP), making him an enemy. Then during combat, drop Withermoore's Soul Jar (from inventory) on the ground and destroy it with a melee attack or spell (1600 XP). This gives you a total of 3000 XP, which is the most you can get from this quest.
