Despite being such a prominent extension to the dark world The Witcher 3: Wild Huntthe Blood and Wine expansion isn't as grim as the base game or Heart of Stone – depending on which ending you get. Whether you've played it before or not, knowing how to get any of the main quest endings available in this expansion will save you the potential headache of having something go wrong over a seemingly irrelevant decision.
Naturally, each of the potential outcomes of Blood and Wine revolves around the fate of two sisters with complicated pasts. Regardless of which path you choose to take, the culmination of events always begins with the “Night of the Long Fangs” quest, when Dettlaff attacks the city with an army of vampires and related monsters. Your choices from here on out will affect the ending in a pretty significant way, so let's go through all the options.

Blood and wine best ending – both sisters live
As you might expect, achieving this ending is the most difficult and requires you to do things in a very specific way. Starts during 'Night of the Long Fangs'search, you must choose to take Syanna to Dettlaff so the story can continue with the “Beyond Hill and Dale” quest.
There are several things you need to accomplish during Beyond Hill and Dale. Make sure first read all journal entries can be found in the journal on the table in the game room. The second is get Syanna's ribbon from the vendor, and finally, make sure you've exhausted all of Syanna's dialogue options before continuing.
The last set of requirements comes in 'Pomp and strange circumstances'search. Here, you must don't forget to visit Syanna before talking to Damien, which starts the ceremony. You will need to talk past the guard and then use the following dialogue options with Syanna:

“Because she turned her back on you…”

“Have you ever thought of forgiving her??”

“He could ask her that.”
Once the ceremony begins and the Duchess approaches you, tell her what you learned about Syanna. She will also ask your opinion of herto which you should plead guilty, but that they should be shown mercy. If you get it all right, the two will finally put the past behind them.
The fate of Regis and Toussaint is another story and then went on to the “Always Be So Humble” quest, but at least this one relationship is a little better.

There are several outcomes that have larger effects when tracking down Dandelion and searching for Dijkstra's lost treasure.
Syanna dies, Anna Henrietta lives
It can be either a neutral ending or a bad Blood and Wine ending (in which case the next ending is downright tragic), depending on how dear you are to Syanna. In the Night of the Long Fangs find Syanna but don't get the tape in 'Beyond Hill and Dale.' This causes Dettlaff to kill Syanna in Tesham Mutna and she and Henrietta will not have the opportunity to see each other.
Geralt will do it they have a chance to fight Dettlaff or not. If Geralt engages Dettlaff, Regis joins the battle – to prove that he is one of Geralt's best friends – and deliver the killing blow.

before that Geralt will be thrown into prison for letting Syanna dieand you will not have the opportunity to undertake the “Pomp and Strange Circumstances” quest – instead, you will unlock 'Housing is the new lane.' Don't worry, your old friend Dandelion will save you.

Regardless of whether you consider this ending merely bad or truly tragic, they exist two ways to achieve this:
The 'Beyond Hill And Dale' Method
If you find Syanna during “Night of the Long Fangs”, you will start the quest 'Beyond Hill and Dale'. While in the Land of a Thousand Fables, Geralt still needs get Syanna's Ribbon from Little Flint Girleither by paying for it or beating her in Gwent. Then there are several ways to reach this conclusion:
- Geralt can refuse to go bootblack when Regis asks him to.

The Invisible Elder Method

On a more positive note, the quest to see the invisible elder is one of the best in expansion although it is easy to overlook if you find yourself invested in Syanna's storyline. This is because you will likely feel the pull to find Syanna during 'The Night of the Long Fangs', which has caused some players to miss the 'What Lies Unseen' and 'Blood Simple' quests.
Fortunately for those of us lacking in wizarding lore, we don't have to miss the side plots of Orianna or Syanna. Players who want every bit of extra story content can feed their souls go to Orianna firstthen he backs up to find Syanna instead of looking for an invisible elder when Regis asks about Geralt's plan of action for the last time near the docks.
Of course, this does not mean that these ends are easily achieved. Both acknowledged worst and best endings they require Geralt to defeat Dettlaff in battle and defeating a taller vampire is no small feat. Dettlaff's power is demonstrated by how much effort he makes Regis fend off his vampires in their various encounters in the Blood and Wine storyline.
How to defeat Dettlaff in his optional boss fight
Dettlaff's optional showdown is one of the ugliest difficulty peaks in The Witcher 3, and even well-prepared players can be shredded if they don't come prepared. Defeat Dettlaff in Blood & Wine, you should be level 45 or higher and wearing a fully upgraded Grandmaster Witcher set — Ursine for survivability, Feline for raw DPS, or Wolven for a balanced approach.
Regardless of armor, keep Quen active all the time; the moment he goes down, Dettlaff's combos can wipe out your health bar in seconds. Yrden helps slow his teleport dashes and gives you a crucial opening.
potionally, Black Blood, Superior Swallow and Thunderbolt can make all the differencewhile Ekimmara Decoction makes your Geralt build a little more sustainable during hit-and-run attacks. In the first two phases, punish his recovery moves with quick strikes, then immediately disengage.
If you want to deal with the ultimate teleportation the hard way, the best technique is to practice patience: dodge to the side, wait for his whip extension attack to finish, and counter with one or two quick hits.
