One Piece's The Final Saga will present some very interesting challenges for the Straw Hat Pirates. This may seem far-fetched to some fans, but Oda has already hinted at it several times in the story. In Enies Lobby, when Luffy pushed beyond his limits with Gear Second and Gear Third, Lucci made it clear to him that it would eventually kill him. In Thriller Bark, when Luffy was using Gear 2 and Gear 3 at the same time, the Straw Hats again emphasized the fact that Luffy was slowly killing himself.
In Dressrosa, Luffy's overuse of Haki clearly put him on death's door. In Wano, Kaido stated this again Luffy overusing his Haki and Gear 5 wanted to kill himwho he told was going to be bones one day anyway so he didn't care. It's clear the signs are there and it's building towards Luffy's terminal illness.
Roger and Luffy's illness may come from overuse of Haki
Fans often wonder what disease did roger haveand Oda gave no answer to that. As all fans know, the disease was incurable and eventually consumed most of his life. Not even a doctor as experienced as Crocus could save him.
Although Oda has not received an official answer regarding this illness, it is highly likely that, like Luffy, he was pushing his body beyond its limits. One Piece: Oda officially confirmed characters with Joy Boy Level Haki
Joy Boy had insane haki in One Piece. These characters have haki similar to or as good as him.
Roger constantly overused his Haki and compensated by not even having a devil fruit. That's why his Haki was the strongest in his time and that's why he was at the very top. However, while Roger's Haki was immense, the body itself most likely could not withstand the constant strain of the incredibly powerful Haki.
This is probably why Roger got sick because his body started to deteriorate and there was no cure for it. Now Luffy does something similar with his excessive use of powerful Haki all the time, with Gear 5.
Chopper saves Luffy from death
It is absolutely crucial to the plot that Luffy gets an incurable disease. Not only does it continue the very storyline that Oda set up as early as Enies Lobby, but it would also give Chopper a way to fulfill his goal.
Chopper wants to be able to cure all diseases. This means that even diseases that are considered incurable will have to be treated. He couldn't save Hiriluk, but he will be able to save Luffy. Once the Straw Hats finish circumnavigating the globe and reach Reverse Mountain again, Crocus and Chopper will most likely work together to save Luffy's life.
