The first step is simple. Just find a bathroom and go to the toilet. In most runs, the doors are already open, so you can enter without too much trouble. The hardest part is that everything is happening around you while you're doing it, because the old man and other dangers are still roaming around the house.
Once there, you must clog the toilet by flushing objects. One large item will usually do the job, or you can use several smaller items if that's all you have on hand.
From what I've seen, a towel is the best option. By itself, it's big enough to flush the toilet without needing anything else, so it saves you a lot of time.
You'll want to be fairly quick because the old man is constantly looking for you and will appear when you least expect it.
After entering the bathroom, I first jumped on the toilet seat and then walked to the washing machine. From there I could get to the sink and get things like perfume and toothpaste. I just picked them up one by one and threw them in the toilet. Once you are done, press the flush button on the left side.
If nothing happens after the first item, you should add more items. You may have to drop a few items before the toilet finally floods. Once you do this, the quest will be complete.
