Obtaining the Mask of the Great Fairy
Not required, but highly recommended

The first fairy
Location: Entrance

The second fairy

The third fairy

Go into the next room with a large mechanical flower in the middle of a poisonous swamp. Go around the tall regenerating Deku Baba by the door and take the wooden path to the left to find another Deku Baba. Put her out of her misery to receive the third fairy.
The fourth fairy
Location: Main Room
There are 5 pots on a platform in the corner of this room and the fourth fairy is in one of them. From the third fairy location, jump over the flowers as Deku Link to avoid the poisonous water and get to the platform. Link will have to go to the platform with the door on the other side of the room before jumping to the corner with the flower pots, since he can't get directly from the flower platforms.
The fifth fairy
Location: Push-Block Room
After getting the first key, go back to the main room where the third and fourth fairies were and go through the locked door. In this room, move the block forward and follow the wooden path to the right, then proceed to kill the Skulltula. Link receives a fairy as a reward for his demise.
The sixth fairy
Location: Push-Block Room
Stay in this room and return to the wooden path to look up at the floating platforms above Link. The hornet's nest on the platform directly above the wooden intersection contains a fairy. Use Deku Link's bubble blast or another projectile to shoot him down. The other nests drop items as well, but only the middle one is required to get the fairy.
The seventh fairy
Location: Dark Boe Room
Above the room with fairies five and six is a dark room with a disgusting amount of black Boes. Eliminate them all to reveal a treasure chest containing the seventh fairy.

The eighth fairy
Location: Main Room

The Ninth Fairy
Location: Golden Eye Switch Room
After defeating the Dinolphos and getting the bow, follow the L-shaped path to see a hornet's nest inside the mouth of the monkey mural that is on the wall to the left of the eye switch. Shoot the nest to release the ninth fairy. The fairy mask calls the fairy to Link for easy retrieval. However, those without the Fairy Mask can shoot the eye and activate the moving platform with the Deku Flower. Now Link can jump down and use Deku Link to get the fairy to jump across the water and then use the moving platform to get back up.
The tenth fairy
Location: Push-Block Room
Without the Fairy Mask, this Fairy was difficult enough to remove in the 3DS version of the game. In the 3DS version, the fairy is in a room with floating Deku flower platforms and dragonfly enemies on the second floor of the temple. The fairy is inside a bubble in the dark northeast corner of the room, in the lower level below the platforms.
For the N64 version, go back to the room with the cube and the wooden path where the fifth fairy was. Now that Link has the bow, he can go to the far end of the room near the Skulltula and look under the wooden path to see the fairy. Use your bow and arrow to shoot the fairy to free her from the bubble. Link will need the fairy mask to avoid damage from the poison water.

The Thirteenth Fairy
Location: Boss Door Room
This one is just above the last one. Continue climbing the alcoves using the Deku flowers until Link reaches the top and the thirteenth fairy is located inside the bubble in the last alcove.
The fourteenth fairy
Location: Boss Door Room
Go back to the main Deku flower located under the ladder and go up the same way as before, but on the left side instead. Another bubble fairy can be seen here. This is the fourteenth fairy.
Again, it's easier to use the fairy mask for those who have it and want to save time. Instead of climbing up the left side of the room, stay where the twelfth fairy was. Then look across the room to see another fairy in a bubble. Pop the bubble and let the mask do its job.
The Fifteenth Fairy
Location: Boss Door Room
After getting the fourteenth fairy, turn around and use the arrow to hit the crystal switch. Once the flames fall, fly straight to the fairy in the bubble. This is the last fairy. Alternatively, the easiest option is to use the bow and arrow to burst the fairy's bubble and then use the fairy mask and have the fairy come right at Link.
Location of the Great Fairy Fountain
Reward claim
After obtaining all 15 fairies, return the stray to the Great Swamp Fairy to restore her to her original form. Her fountain is located in a cave near Woodfall Temple. It can be seen from the altar where Link raised the Woodfall Temple and can be reached by flying over as Deku Link.
